Seniornest is seeking a dedicated and compassionate Clinical Liaison to join our team in Paramus, NJ. In this role, you will serve as a vital connection between healthcare providers, patients, and families, ensuring seamless communication and coordination of care. Your expertise will help guide individuals and their loved ones through complex healthcare decisions, providing support and clarity during challenging times. Responsibilities

  • Act as the primary point of contact between patients, families, and healthcare professionals.
  • Coordinate referrals and facilitate transitions of care to appropriate senior living or healthcare resources.
  • Assess patient needs and provide education regarding care options and services available.
  • Collaborate with clinical teams to develop comprehensive care plans tailored to individual patient needs.
  • Maintain detailed documentation and follow-up to ensure continuity and quality of care.
  • Represent Seniornest professionally in meetings, outreach events, and networking opportunities.
Requirements
  • Relevant clinical background, such as nursing, social work, or related healthcare experience.
  • Strong interpersonal and communication skills with a passion for patient advocacy.
  • Ability to navigate complex healthcare systems and community resources.
  • Excellent organizational skills and attention to detail.
  • Valid driver's license and reliable transportation for local travel.
